2008 VIETNAM MEDICAL MISSION TRIP

Introduction:

The annual Vietnam Medical Mission Trip is a non-political, a collaborative humanitarian effort among Hope Initiative, University of Houston College of Optometry, and Baylor College of Medicine to provide medical relief and humanitarian aids for the unfortunate people and orphans in various region of Vietnam.  This program provides the unique hand-on experience in working with doctors, optometrists, and other volunteers to affect direct assistance to Vietnamese people.  You will find below general information on this year trip.  For specific question, please contact the designated coordinators.

Trip Length:  November 28^th, 2008 to December 13^th, 2008

Expenses:    $1,125 - airfare & visa

                    $650 - room/board plus 2 meals/day

**Note:     HI will purchase airplane tickets, obtain necessary visas, and arrange necessary transportation, hotels, and accommodations on your behalf.  However, you are responsible for all expenses as listed above.  You may elect to purchase your own airfare.  If you choose to do so, please contact Tung.Tran@hionline.org.

Requirements:

To be considered as a participant on the 2008 Vietnam Medical Mission Trip, you must:

1. Be a member of Hope Initiative
2. Download all documents below 
3. Fill out the Medical Mission Trip Application with all required information and return it to Tung.Tran@hionline.org by September 12^th, 2008
4. Print, Sign, and Return the Waiver and the Code of Conducts Agreement to the coordinator by November 28^th, 2008
5. Submit a non-refundable deposit in the amount of $300.00, payable to Hope Initiative, (memo Medical Mission Trip 2008) to Hope Initiative’s mailing address  by October 1^st, 2008

Sponsorship:
